Not closing by manual operating actuator piston. Air actuator system found leaking and not functioning. Additionally found spring loaded valve on level gauge permanent blocked in open position by device. 3.Safety management audit by the Administration is required before departure of the ship. Emergency generator-cooliing fan duct canvas deteached; starting control panel (manual and auto mode included) switched off. 2. Ship main boiler-flame eye sensor out of order. 3. Incinerator-the system completely out of order. hip's crew not familar with how to operate incinerator completely. 4. Jacketed F.O. high presure pipes for main enging-leakage detection alarm box disconnected with leakage drain pipes. 7 1. PSCO found port and stbd. Life rafts tied down preventing float free arrangements. The cover for davit life raft sling was compromised exposing the life raft to the elements. Markings required by the LAS code were missing on the life raft containers including length of painter, and maximum permitted stowage height. Discrepancies identified in the packing/servicing of life rafts. Stern launch lifeboat missing seat restraint. 2. Automatic sprinkler system was found in manual operation rendering system not able to start automatically as required. 3. PSCO observed 25 minute delay in delivery of water to the hydrant after starting fire pump. Double was found port side Fwd of the fire main. Leaks were observed on hydrants and piping. General service and emergency fire pump wiring to the motor had conductors not insulated and strain relief was not provided to conductors. 4. Diesel oil leaks were observed on bulkhead fittings from the storage tanks into engine room. Leaks was observed at 1 drip/2 seconds. Additional leaks were observed on fuel oil piping under purifiers. 5. Hatch covers were found with modification/unauthorized repairs. Sounding tubes for voids and ballast tanks were missing caps allowing down flooding. Ventilator of hatch covers were severely wasted preventing proper closure. Anchor wash was holed, allowing water into Bosun store. 6. PSCO observed leaks on #1 and #2 generator saltwater inlet pipes in way of temporary repairs presenting possibility of overheating and loss of 2 out of 3 generators. 7. PSCO observed limited access to fire suits located at main deck stairwell. Foam applicator and containers in upper level engine room by boiler were stored in a position not available for immediate use. 8. Lifejackets and immersion suits for 6-man liferaft in bosun store were stored in a manner not ready for immediate use. 9. All crankcase explosion covers were excessively coated with paint preventing proper function in event of crankcase explosion. 10. PSCO observed all ships service diesel generators, and OWS bilge pump missing machinery guars presenting hazard to crew. 11. The fuel supply system panel for generators is not functioning as designed. Several gauges and switches were disabled and rendered inoperable potentially leading to unnoticed generator fuel service tank loss. 12. Tank tops for #2 sludge tank and bilge water holding tank were not bolted. Tank top bolts were reported removed in October 2020. no bolts were present and covers were not secured. Fuel leaks accumulating in bilge are disposed in sludge tank. 13. Self-closing a-60 fire door in lower level engine room for emergency escape failed to self-close and is damaged. A-60 self-closing fire door on deck c ladder well did not self-close as required. A-15 door for galley found damaged and ineffective. 14.
